What to Expect (Job Responsibilities):
- Conduct telephonic outreach to Medicare members to improve medication adherence and health outcomes
- Utilize motivational interviewing techniques to assess members' health status and promote behavior changes
- Collaborate with members to identify and integrate them into available internal and external health programs
- Provide coaching and support to empower members in making informed healthcare decisions
- Develop proactive action plans to address health-related issues and enhance overall wellness
What is Required (Qualifications):
- 1-3 years of experience with Microsoft Office
- 3-5 years of practice experience in areas such as Renal, Diabetes, Hypertension, Primary Care/Geriatric, or Oncology
- Experience in Managed Care and Medicare
- Associates Degree or equivalent experience
- Strong analytical, problem-solving, and communication skills
How to Stand Out (Preferred Qualifications):
- Healthcare licensure and/or certifications (RN, LPN, MSW, LCSW, CDE, etc.) highly preferred
- 5 years of practice experience in relevant healthcare fields
- Previous experience in telephonic coaching, education, or case management
- Excellent time management and organizational skills
- Bilingual skills are a plus
