What to Expect (Job Responsibilities):
- Practice in accordance with state laws governing advanced practice nursing or physician assistants
- Establish and maintain effective relationships with patients, caregivers, and other healthcare professionals
- Perform initial comprehensive assessments on newly enrolled patients and provide ongoing care
- Prescribe appropriate diagnostics and interventions to avoid unnecessary acute admissions
- Educate patients on disease processes and ways to manage them effectively

What is Required (Qualifications):
- Completion of Nurse Practitioner or Physician Assistant accredited program
- Current state RN license
- Current NP or PA license (PA, ANP, FNP, or GNP credentials)
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems

How to Stand Out (Preferred Qualifications):
- Knowledge of CPT, ICD-10, and HCPC codes
- Knowledge of clinical standards of care
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
